Appendix Cancer: Rare but Not to Be Overlooked

Primary appendiceal tumors are uncommon malignancy arising in the appendix and found in less than 2% of surgical removed appendix (Arch Pathol Lab Med 2011;135:1261) and rare, often unexpected, and sometimes difficult to diagnose. Because its symptoms can resemble acute appendicitis, some cases are discovered unexpectedly following an appendectomy.

🔬 Why is accurate diagnosis important?

Appendiceal tumors include several distinct histologic types, such as:

  • Appendiceal adenocarcinoma
  • Mucinous neoplasms
  • Neuroendocrine tumors (NETs)
  • Goblet cell adenocarcinoma (GCA)

🧬 The Role of IHC

Immunohistochemistry is a reliable tool in the distinction between primary ovarian tumors and metastases from appendiceal oringin and helps pathologists determine tumor lineage and origin.

  • SATB2 – is a sensitive and specific marker of colorectal epithelium, also positive is appendiceal epithelium. This marker has shown 93.8% sensitivity and 97.5% specificity in determining appediceal oringin (Histopathol 2016;68:977).
  • CK20, CDX2, and MUC2 – are also optical markers of appendiceal origin, although they are frequently expressed in primary ovarian tumors, such expression is usually patchy and focal. Diffuse and strong expression for CK20, CDX2, and MUC2 has 90%, 87.5%, and 95% specificity for appendiceal origin.
  • PAX8 – 0% positive in appendiceal origin, but 30%-65% of primary ovarian mucinous tomors and 80% of primary ovarian emdometrioid tmors.
  • p16 – Strongly and diffusely positive in the metastasis of cervical carcinoma, but is negative in appendiceal oringin.
  • ER – 0% positive in appendiceal origin but 30-50% of primary ovarian tumors.
  • Synaptophysin and chromogranin – supports neuroendocrine differentiation, important because appendiceal NETs are a major appendiceal tumor category.

Accurate classification matters. IHC provides valuable diagnostic clues that help pathologists reach the right diagnosis and guide appropriate patient.
